If you want still more room, you can choose between medium (1D), wide (2E) and extra wide (4E). That is a real difference from shoes that come in a standard width only: instead of taking a half size up and sliding around inside, you simply pick the width that suits your foot.
Good to know
This is an outspoken max cushion shoe, not one for track work or a fast 10 kilometres. If you are coming from a shoe with a high offset or a thin sole, expect the first runs to feel different: there is far more material under your foot and the offset is low. Yes, the shoe has a spacious fit that allows for it. You take the original insole out and put your own in. Because there are also three widths available, you can pick the width that suits the combination of your foot and your orthotic.

Medium (1D), wide (2E) and extra wide (4E). If you are unsure, it helps to know that the Ghost Max 4 runs more generously than a classic running shoe anyway. If a standard width normally feels just a little tight, medium is usually enough; if you have a genuinely broad forefoot, go for wide or extra wide.

That is the essence of a max cushion shoe: the more foam between you and the asphalt, the more of the landing is absorbed.
